Day Camp Registration

 

Get Ready to Register

 

There are several steps you should complete before beginning to register. Please look over this page--you may not be able to complete your registration without collecting information described here. This is very helpful for the paper application process at the YMCA.

 

IMMUNIZATION RECORDS: Your child's doctors office may be able to provide you with these records if you cannot easily locate them. 

 

PHYSICIANS INFORMATION: Be sure to have your child's doctors name, doctors phone number, Insurance company and Policy # on hand.

 

FAMILY & FRIENDS: There is a special place on the application for family and friends names and phone numbers. In case of emergency or someone to just help pickup your child/children. Be sure to have their home, work, and cell phones numbers on hand.  Regarding custody or restraining issues we will need Legal Documentation (if applicable).

 

PAYMENT: You will be asked to pay a $40 registration fee per child. (Non-refundable. Financial aid does not apply to this fee.) The full payment for the first week and your choice to pay a $15.00 deposit per week to reserve space. See Rate page for more information. 

 

REGISTRATION FORM: To register, you must complete a Camp Registration form (also available at any YMCA registration desk) prior to your child attending camp.

MEDICATION FORM: Order for Medication (Required for children requiring medication during the camp day)

ATTENTION!

Parents with last names different than their child's, please mention the child's last name in any communications with the membership office, such as payments etc.
